1095-C: Benefits Administration sends an annual email notice advising state employees that their digital copy of the 1095-C form is available to view in Edison. Benefits Administration only furnishes and distributes 1095-C forms for state employees who were offered health insurance during the previous year.
Higher education, local government and local education agencies are responsible for furnishing and distributing their own 1095-C forms to their employees by the published IRS deadline.
1095-C forms for state employees are placed in Edison by early March for the previous year.

W2: If you are an employee of the state of Tennessee, you can find your W2 form in Edison using the steps below. If you are NOT an employee of the state, you must contact your employer for your W-2, as they are not in the Edison system.

1099-SA / 5498-SA / IRS Form 8889: The 1099-SA form is used to report distributions made from a health savings account, or HSA. This form cannot be found in Edison. Which forms do I need to file my taxes?
There are three tax forms associated with health savings accounts:
- IRS form 1099-SA shows the amount of money you spent from your HSA during the tax year.
- IRS form 5498-SA shows the amount of money deposited into your HSA for the tax year.
- IRS form 8889 is the form you fill out and submit with your tax return.
Please use the information in your 1099-SA form, available online, to fill out IRS tax form 8889. Form 8889 is the only one you need to submit with your taxes. You can find IRS tax form 8889 in the on the state’s HSA administrator’s website in “Statements & Tax Docs” section after signing in to your account.
When will I get my tax forms?
IRS Form 1099-SA is typically available at the end of January. It will be posted to your account and mailed, if elected. IRS Form 8889 can be downloaded from IRS.gov at any time. If you contribute in the new year for the previous tax year, you will also get another 5498-SA form in May.
