How do I cancel coverage in The Tennessee Plan (Medicare supplement)?

The retiree (or surviving dependent who became head of contract) must submit a written request to cancel coverage. The written request should be sent to Benefits Administration by mail, fax or email. Coverage will be terminated the first of the month following receipt of the written request.
