For some drugs, there may be a post-quantity limit authorization available. Your doctor will need to contact CVS Caremark at (800) 294-5979 (this phone number is for doctors and their staff only) and provide clinical information to request an amount over the plan limit. As the plan’s pharmacy benefits manager, CVS Caremark will review this information and decide if the member’s insurance plan should cover the amount above the limit. Some drugs have a set quantity limit, and the plan will not pay for a quantity beyond that. Anything over that limit must be paid by the member.
