Health Insurance-COBRA
When an active employee dies, the surviving spouse and surviving dependents who are on the employee’s active health insurance at the time of the employee’s death, will receive six (6) months of free health coverage. At the end of the six months of free health coverage, the surviving spouse and surviving dependents are eligible to enroll in COBRA health insurance for up to 36 months as long as the surviving dependents remain eligible dependents. In order to enroll in COBRA medical insurance, the surviving dependents must enroll before the end of the six months of free coverage.
Dental Insurance-COBRA
When an active employee dies, the surviving spouse and surviving dependents who are on the employee’s active dental insurance at the time of the employee’s death will be eligible to enroll in COBRA dental insurance for up to 36 months as long as the surviving dependents remain eligible dependents. In order to enroll in COBRA dental insurance, the surviving dependents must enroll within 60 days of the active coverage ending.
Vision Insurance-COBRA
When an active employee dies, the surviving spouse and surviving dependents who are on the employee’s active vision insurance at the time of the employee’s death, will be eligible to enroll in COBRA vision insurance for up to 36 months as long as the surviving dependents remain eligible dependents. In order to enroll in COBRA vision insurance, the surviving dependents must enroll within 60 days of the active coverage ending.
