Durable medical equipment is subject to deductible and coinsurance. For in-network services, members are responsible for the following after the deductible has been met:
Premier PPO 15%
Standard PPO 20%
Limited PPO 30%
CDHP/HSA 20%
Local CDHP/HSA 30%
