If you have elected to enroll a new dependent into coverage during Annual Enrollment, you must also submit dependent verification to verify that the dependent you have elected to enroll is an eligible dependent. For Annual Enrollment All dependent verification must be received by Benefits Administration before December 1 of the current plan year unless otherwise instructed by Benefits Administration.
The list of acceptable dependent verification can be found by clicking here, choosing Publications, then Forms. Then you can navigate to Active Employees and COBRA and choose the dependent verification document from this list.
These documents can be submitted in Edison self-service after you click the "Submit Enrollment" button on your benefits enrollment. You can also upload your documents at the bottom of the Dependent Information tile. This is the same tile where you add your dependents’ demographic information. You can also submit by email to benefits.administration@tn.gov (if emailing please include Edison ID and your first and last name) or by uploading directly in Zendesk by clicking here.
