Benefits Administration requires a Social Security card to change names.
If your last name, or the last name of one of your dependents, has changed due to a marriage, divorce or other circumstances, please contact your agency benefits coordinator to change the name on insurance enrollment and provide them with a copy of the Social Security card of the person whose last name should be changed.
