Benefits Administration now requires a Social Security card to change names. This is in compliance with the requirements of the Patient Protection and Affordable Care Act. Benefits Administration is required to submit information on all persons covered under our plans, and their names must match federal records.
If your last name, or the last name of one of your dependents, has changed due to a marriage, divorce or other circumstances and you are not also submitting an enrollment change request:
Please provide your agency benefits coordinator with a copy of the Social Security card of the person whose last name should be changed, and ask them to submit it with a Correction and Clarification form to Benefits Administration.
