If you have recently adopted or been awarded legal custody of a child for the purposes of adoption, an application to add the newly acquired dependent must be submitted to Benefits Administration within 30 days of the acquire date to obtain coverage from the date of adoption or placement, or within 60 days to obtain coverage from the first day of the month after submitting the application.
To add a newly acquired dependent to your State Group Insurance Program coverage, fill out an Enrollment Change Application. You can find the application on the forms webpage at https://www.tn.gov/partnersforhealth/publications/forms.html.
Also on the forms webpage, you will find the Certification for Eligibility for Dependents Placed with Head of Contract by Valid Placement Order.
You are also required to submit a copy of the signed court order of adoption and order of placement for adoption if applicable, along with the Enrollment Change Application to your agency benefits coordinator, or ABC.
NOTE: The State Group Insurance Program does not prorate premiums. If the effective date due to adoption or legal custody for adoption is a date other than the first of the month, you will be required to pay the premium for the entire month.
