Children covered under a (ex-)spouse's plan can be added if the spouse's coverage is lost, even temporarily. The child can also be removed as a dependent under your plan if your spouse regains coverage.
To add a child as a dependent due to loss of other coverage, submit the following information to your agency benefits coordinator:
- Enrollment Change Application: https://www.tn.gov/content/dam/tn/finance/fa-benefits/documents/2022_forms/1043_2022.pdf
- The child’s birth certificate (will accept mother’s copy for newborn)
- OR Certificate of Report of Birth (DS-1350); OR Consular Report of Birth Abroad of a Citizen of the United States of America (FS-240); OR Certification of Birth Abroad (FS-545)
To remove the dependent child from coverage
Once your child(ren) are back on your ex-spouse’s insurance, you can then remove them from your coverage by submitting the following information to your ABC:
