Correction Required by Benefits Administration
If a correction to the date is needed, a Benefits Administration Analyst will make the update. However, you must take the following steps to initiate the correction:

Please complete and submit the Corrections and Clarifications Form in one of the following ways:

Submit the completed form in one of these ways:
- Through your Agency Benefits Coordinator (required for signature)
- Or by submitting a Zendesk ticket with the correct information
Important: The form must be signed by the Agency Benefits Coordinator before submission.
